By taking either of the Investigator Access Quizzes you are acknowledging the following statement:
"I have read the continuing education course booklet containing information about alternatives to the use of live animals, variables brought about by using animals and how they can be controlled, surgical requirements for rodent and non-rodent species, the use of analgesia, and anesthesia, proper methods of euthanasia, hazards associated with the use of animals and reporting animal incidents. Other information associated with the writing and processing of protocols, ordering animals, per diem charges and the functioning of the Institutional Animal Care and Use Committee were included in the booklet.
I also understand that this type of instruction is to be an annual occurrence."
• Caretaker Quiz (for PAR husbandry staff)
By taking the Caretaker Quiz you are acknowledging the following statement:
"I have read the Program for Animal Resources employee handbook. This handbook details the duties and responsibilities which my position covers, as well as safety information related to the environment in which I will be working. I also understand that this type of instruction is to be an annual occurrence."
When you have finished taking your quiz, if you have not already done so, please fill out the Occupational Health Program Enrollment Form and return it to the PAR manager.
